The Central Registration office now supports registration for all of our ChattCo schools (Pre-K thru 12th grade) in one location at 326 Broad Street, Cusseta GA 31805. The office will be open Monday-Thursday 8am-12pm. Contact our Registrar, Claudia Adams at cadams@chattco.org Items needed to complete Registration: Birth Certificate Social Security Card Parent/Guardian Photo ID Guardianship-if applicable GA Form 3231 (Immunizations) GA Form 3300 (Ear, Eye, Dental, Nutrition Screening) Residency Affidavit (Notary not required) Proofs of Residency 1. Lease or Proof of Ownership 2. Utility bill, Military Orders, Bank Statement, etc.
